I have ltd benefit from my emplyer after my waiting period i applied for it obtain form from employer and after when i fill it i send back to employer but my emplyer put wrong information about my health i also send dacoment regard to my health from a surgen who done my surgery how they don;'t believe they just believe the information that the employer give to them but truth is that my emplyer never call regard to my health i just want to know now how my doctor help me how and what i say to my doctor? can i wrote a letter to my doctor who is taking care of me after accident and what i say him

First get a doctor's letter confirming your disability. Then write to the insurance company and request a review. Send a copy of the doctor's letter to them and copies to your companies Human Resource department and the Ombudsman of insurance for the Province of Ontario. Also, if you were injured on the job, you are eligible for Worker's Compensation. Apply to them directly.
