If this is what you forgot to do and will be doing it after 15 years, you can approach your lender and find out if they have it in file. They would also point you in the right direction where to get the document in case they do not have it in file. It has to be available somewhere.

ON Canada - lenders normally send notifications 3 months before the renewal date. You may even get a call from the bank about setting an appointment for you to come by and sign renewal documents.
